Does it only come in an assembly or can it be installed alone?

Price depends on where you get it; shop around. Before you change it though, make sure it's the motor and not the electronic control unit (I had to change one in my 94 Astro) which is cheaper easier to change. The motors are usually pretty rugged and seldom go bad, so one from a salvage yard is a good option at a much less painfull price.
It will run in the range of 75-85 dollars depending on the place of purchase. It comes alone and is very easy to change on this model vehicle. One thing is to be sure to take the old one off and take it with you to the place of purchase to compare the motors. There are two different types for this year. I would also check some salvage yards, there cheaper and some places already have the part removed or will remove it at your request. Most of the salvage yards give a 30 day warranty at a third of the cost.
I JUST LOOKED IT UP FOR YOU YOU CAN EXPECT TO PAY IN THE AREA OF $75 FOR THE WIPER MOTOR ASSEMBLY THAT IS IF YOU HAVE YOUR OLD ONE WITH YOU WHEN YOU BUY IT IF YOU DONT, EXPECT TO PAY ANOTHER $30 FOR A CORE CHARGE WHICH YOU WILL GET BACK UPON GIVING THEM THE OLD UNIT IT IS SOLD AS AL ASSEMPLY BUT THERE MAY BE A PIECE OR TWO THAT YOU WILL NEED TO SWITCH TO THE NEW ONE
