Question:

Can anyone identify this boiler model?

but it's way too damn blurry for me to make anything out except for 125,000 input BTU and 100,000 output BTU and it's made by Slant/Fin and it's a water boiler.

Answer:

Slant/fin GG-125
It is a gas-fired, single-fire (non-modulating) hydronic boiler - meaning that it burns natural gas to heat water to distribute via a circulator pump to radiators or some other form of radiant heat. Gross input is 125,000 BTUs per hour, or 125 cubic feet or 1.25 therms per hour. Net output (delivered to your heating system) is 100,000 BTUs per hour, making it 80% efficient. Its maximum working pressure if configured for water is 30 psi. For steam (unusual, but possible) it is 10 psi. You will be able to tell the difference. A steam boiler will have a sight-glass to show boiler water level. A water boiler will have a circulator pump. Information on the latest version of this boiler may be found at the link below. Slant-Fins tend to be reasonably made units with decent longevity if maintained. However, this is not a very efficient unit as they go these days.
