I've never heard of that, but it would be up to the person giving you the form. It's probably better to go to an internal medicine doctor, preferably your primary care doctor's practice if possible. They know your health history, can get the ball rolling with any questions/concerns you want addressed, and request blood work if appropriate. But most importantly, you would get a more thorough heart exam (rhythm, murmurs, etc...) and lung exam (clarity, wheeze, etc...) since these are the organ systems we examine all day.
