How do I connect CAT5 wire to a RJ45 wire?

I have a wire coming up from my basement that has 3 wires (red, green and yellow). I want to connect cat5 wire to that (twisting the wires together) and run the cat5 to an RJ45 box. What's my diagram going to look like?

What will you be doing with this setup? the three wires you talk of are telephone service wires cat5 although it can be used for telephone, is for computer network service. RJ45 being the jack for ethernet networking. RJ11 is the jack for telecommunications.
the wall plate has been terminated at TIA 568 b so that you only want to be sure out in the journey that your modem is waiting for the cable as TIA 568B or TIA 568A.. there's a huge difference because the pairs are terminated in a unique way. seem at reference I appropriate .. i do not use those type of wall plates I perfer to apply the punch down form and then punch them down as TIA 568B .. i have self assurance the issue you're having is interior the way the wall plate is operating.
if you are using for phone, then you will just need to be concerned with green/red and blue/blue white for line 1 and yellow/black orange/orange white for line 2 line #1 greenblue/white redsolid blue line #2 (not required to connect if you only have 1 phone line) yelloworange/white blacksolid orange polarity is not important with modern telephone equiptment, so as long as G/R=BL-BL/W and Y/B=OR-OR/W then it does not matter if the striped wires are reversed from the list above... seeing your question again.... there is a wire missing, if you strip back the outer sheath of the wire you should find a black wire as well.... but green/red is the important pair for line #1
Cat5 requires 4 wires (two pairs) - you cannot use old telephone wire. second, if the old phone was was 2 pair, you still could not use it. Cat5 is good to just over 300MHz. The old voice grade JK cable could not transfer data very fast. Just use the old phone wire as a pull. Connect a new run of Cat6 to the old wire and pull the new wire into place using the old wire as a pull string.
