Intubation and mechanical ventilation is necessary. I don't do cardiac anesthesia, but when I trained a million years ago, we used mostly opiates and only a little inhalation agent. There are also a LOT of pressors and inotropic agents used. The short version of the surgery: Prep, drape, cut through skin and saw through the sternum. Expose the heart, and cannulate for bypass. The bypass machine takes over the work of both the heart and the lungs, so we turn off the ventilator and just watch the flat arterial line tracing. The heart is stopped with cardioplegia solution (lots of potassium in that) pumped through the coronary vessels, and the patient is cooled in many cases. Surgery (coronary revascularization, valve replacement, whatever) is done. The patient is warmed, the heart restarted, and everything is closed up. The sternum is wired closed. Pacing wires are usually left in place for a few days in case the heart chooses to something funky. Patient then goes to the ICU intubated and ventilated for a while, on whatever drips are required to keep the heart pumping and blood pressure compatible with life. I know there are advancements with less invasive procedures and such. Many patients also have transesophageal echocardiography to evaluate heart function before and after the procedure.
