Question:

How would outside personnel be alerted that an ICBM was going to be launched in the event of an attack?

If it came down to a nuclear attack, how would the personnel working around a missile silo know to leave and how far would they have to go to take cover from the missile exhaust blast?

Answer:

If a silo is about to launch a missile, they wouldn't worry about their own people being hurt because they aren't being targeted by the missile. The missile fires from underground, so the exhaust is less likely to harm anyone above ground. Beside that, nuclear silos don't have many personnel above ground because they are disguised. That also makes them less likely to be targeted. However, if they DO get targeted, they would not have much warning, which it would be pointless to try to escape from. I am pretty sure the facilities themselves are incredibly resilient bunkers though, so they would probably do better to stay inside the facility.
If it came to a situation where an ICBM could possibly be launched, there probably would be no one int he blast zone working. And if there were, they would be one of the last concerns if launching.
In event of an attack, there would be no warning other than the blast doors being blown off the silo Its not like there are people working above ground outside a silo.
They don't. The only alert would be streaks of fire and smoke heading into space. Minutes later, opposite streaks would be falling proceeded by a blinding flash and then Armageddon. US nuclear silos would be some of the first targets, to try and destroy them before they're launched. Most people working there/on the base, would die or would not survive unless they're underground. No point in running, you can't escape far enough away via ground vehicles with so little time. The only known procedures for nuclear attack is this: 1. Grab your ankles 2. Put your face between your legs 3. Kiss your a$$ goodbye
