1) Firearms DO have spread. If you were to mount a rifle in an extremely stable vise pointed downrange, and start firing single shots into a target 100 yards away, you would get a group. The group might be 1/2 in diameter, or it might be 4 in diameter, or it might be anywhere. Change ammunition to a different brand, and you'll likely get a different group. 2) If you tried to fire any gun from the hip, you'd probably have a bear of a time just trying to hit the broad side of a barn from any decent distance, much less a human. You'd have to be a gyroscopically stabilized, very strong and heavy robot to effectively counteract the recoil and mass of the firearm. 3) If you get your information from video games, don't expect your information to be realistic in the least. Video games are not the real world, else everyone would go around sleeping with prostitutes to get their health back, then running over the prostitutes and getting their money back (yes, that happens in some of the Grand Theft Auto games, which I do not play). EDIT: Just to clarify, this answer is based on real life. If you are going to create a video game, why are you asking questions about REAL guns? Video games are popular because they are NOT realistic. If video games were like real life, you would never make it very far, because the first time a bullet hit you in the arm on the game, you'd be out of the game. There is no need to admonish us for pointing out how ridiculously unrealistic video games are. If you want a video game answer, ask in the video game section. It doesn't matter to me if you're a teenager or a 90-year-old. I'm answering your question based on real life either way.
