Um............. No you can't, but you can change their appearance with a mirror tho
If you have the university expansion pack you can. If you go to university, graduate, get the superstar career, once you have gotten a couple of promotions it will unlock your career reward and its a platic surgery station.
You need the Sims 2 University Expansion pack for that. There you will use the cheat unlockcareerrewards and then go to the career rewards panel and find the plastic surgery tool. Make sure your aspiration is at gold level or above or your sims will come out fugly!
Plastic Surgery is a career reward for the slacker career (the one when you can become a movie star) There is actually 2 or 3 options on it that you can choose from on it. The first one you dont actually get a choice on how your face ends up, the other one you can totally redisgn your features
