Driver‘s side wiper works fine at low speed but at med and fast speed it gets over-enthusiastic and ends up off the windshield. How do I fix it?

Remove it from the spline and place it back on the spline in the new correct position.
tighten the bolt that holds it in place, likely it's a compression fit wiper, and it's just kind of loose.
