Different mix. Concrete has more gravel in it as rock is harder than concrete and cement is more a mixture with sand. Quite interchangeable language on the jobsite like Tomato or tomayto. They generally know what you want so they bring it. I don't doubt that the lime content changes but I will leave that secret up to them.
Cement is the gray powdery stuff that comes from a natural stonish product called 'klinker'. It is just ONE element to the concrete. To make simple cement stronger, you go for concrete: add sand, gravel and in the best of cases, rebars (iron rods) - that way the strength of both major elements are combined: 1- Concrete by itself is super-strong for compression, but cracks when pulled or bent (inflexible). 2- Iron/Steel is extremely tough to pulling forces and to some extent flexible. Combine the two and you got the stuff many skyscrapers are of.

Cement is the stuff that binds the aggregate together. A common type of cement is Portland Cement. The stuff is like sand. It's not hard and it can come in bags. You can pour it out. When you mix cement with water and aggregate then let the mixture set, it becomes hard, almost like stone. It becomes concrete. After it has hardened, you can drive a tractor trailer over it or build a fairly tall building on top of it.
