his safety everyone else's safety is the main concern here. he needs help no insurance is not excuse to let him continue to walk around dousing clothing w/oil threatening to kill..you need to encourage him to CHECK HIMSELF IN somewhere. they will hold him 72 hrs evaluate him regardless of if he has insurance or not. when they see that he is a risk they will hold him longer. in the meantime i'd contact a counselor seek out more info in your state about getting help for someone mentally ill that has no insurance. there may be some programs that can help.
I mean if you are really concerned go to the person first. All OSHA is going to do is fine the plant and possibly get the worker fired. If you are not a Foreman or a Supervisor etc then i would mind to myself. If you wanna be a rat then do it, but you are risking getting your *** beat by unhappy workers.

call the police they can have him admitted to a hospital for a 72 hour hold because he is a threat to himself and his family, if he needs to be in longer they will make arrangements for that.
