All insulation should be covered up in any areas where you will be spending time. It's caused by incomplete combustion. Basically, anything that burns can produce some carbon monoxide. Gas stoves or furnaces, wood fireplaces, candles, oil lamps, cigars/cigarettes, etc. The best prevention would be to minimize combustion inside the home. Have gas furnaces and water heaters cleaned and serviced periodically. Buy a CO detector or two.
