I am converting from oil to gas and have a gas line already in the house. This includes everything except the boiler, including removal of oil tank and boiler. The basement is easy to work in.

Yes $4500 sounds fine. As long as the gas installer person is legally certified to work with gas you should be ok. Whether it be a combination boiler, an open flue or balanced flue boiler $4500 sounds spot on. You should know that combination boilers are the least reliable so make sure you have a trusted gas worker to fix the boiler if it ever goes wrong. I am a gas worker trainee so I should know!
