Blend doors are out of whack. You may have a vacuum hose broken or off that control the blend doors.
It's OK for your defrost air to be slightly cooler than the warm, heated air blowing out of your dashboard vents. Warm, moist air blowing against the inside of the cold windshield glass will add frost to the glass, not remove it. Most modern automobiles use the car's air conditioning system to dehumidify the windshield defrost air so to make it more effective in keeping the windshield clear. You can check this by listening to the sound of your engine as you switch the heater controls from heat to defrost... You will hear the A/C compressor kick in when you move the control over to defrost.
Your Heater core has most likely gone bad and needs replacement , the heater core is a radiator like device used in heating the cabin of a vehicle. Hot coolant from the vehicle's engine is passed through a winding tube of the core, a heat exchanger between coolant and cabin air. Fins attached to the core tubes serve to increase surface for heat transfer to air that is forced past them, by a fan, thereby heating the passenger compartment.
