From a home stereo receiver there is a sub woofer output what kind of cord/connector is this?

it is normally called an RCA, but to be more precise and specific it is called a subwoofer cable. a normal RCA cable doesn't normally include a y-splitter, but an RCA Subwoofer cable usually comes with it. bestbuy, walmart, radioshack have them.
It is called RCA jacks, they are single RCA jacks on each each. You can find them at most retail store like Best Buy, Walmart, Target, Fry's Electronics and Radio Shack.
