Any tips on how to wash smoke-stained ceilings without breaking your neck?

I have to help clean these horrible ceilings - coated brown with decades of gross tobacco cigar smoke! Is there an easier way than standing on a ladder and breaking my neck washing straight up over my head? And what would a good cleanser be for this? Please don't tell me to just paint over them, as they still need to be cleaned before paint. Any shortcuts, tips, advice is much appreciated!

the lock part of that one can be put back in place on it without buying a new one for it,i own two of those cars and probably all that has happened to it is the holder for the lock assembly has fallen off it,that can be replaced,good luck.
