Do you insulate entire ceilings?

A friend in the Philly area is renovating his house and wants to know if when you're insulating the first floor ceiling, do you insulate it completely or do you leave a quot;heat zonequot; in the middle of the ceiling to allow for heat transfer to keep the second floor's floor warm in the winter? To me, putting a hole in the middle of the ceiling without insulation doesn't seem very logical but who am I? hah Thanks everyone.
