I agree this is not a safe environment for the kids to be in.. I would talk to your husband about it first though and make sure the two of you are on the same page due to the fact that it is your husbands co-worker.
I would call CPS it is obvious that the woman needs help with the twins and house management. Being a mother of twins myself it is difficult but can be done if she is willing to accept offers of help. She may be taught to be on the look out for illnesses and help with various other problems I found having rubber sheets or mattresses protectors really helped I had joined a group in the community with multi children that was a blessing and the input of information was great. Occasionally i let things go but, didn't let it build up where i could handle it, she may be suffering from post par tum depression or some other physical illness I would see it you could help her find a helper or houekeeper/nanny that comes in once or three times a week. to help.
