So happy you went to a lactation consultant. They can be so helpful! I also used a nipple shield for a few weeks with my daughter, and it was tough to figure it out in public! I'd suggest practicing at home, over and over, so that you'll be a pro soon enough! Try using a clothes pin to attach a receiving blanket to the clothing on your shoulder for more privacy. Also, if you have baby in your left arm, you can use that same hand to hold a receiving blanket over baby's head so that you might be able to get the shield on and baby latched. While you are getting everything done, don't worry about what people around you are thinking, just focus on what you are doing so that you don't get stressed.
