Basically, what is it supposed to do?

Answer:

It is up to the employer to enforce the rules in their company. If they weren’t doing so then they are as guilty as the employees who committed the violations. If I were to walk in my office with a skirt 2 inches too short, you bet my employer would say something and will advise for the incident not to be repeated or consequences will follow.
Because unionised mines do not have accidents like this, and are much safer, and better-kept overall. This company has a history of violations; they can make more profit just paying the safety-violations than actually spending hundreds of thousands of dollars to comply with the safety-standards. Capitalism rocks! Yeah!
stereotype, i think. just like how many accountants are portrayed as being of Asian origin, and waitresses in bars always seem to be portrayed as thin and beautiful.
Actually, safety violations may be the result of employees, but 3000 in 10 years is a lot. That's 30 a year! My company has not had a reportable safety violation in over 2 years! NOT ONE!!! No offense, but if you had ever worked in an industrial sector, you'd know why some are blaming the company. Not only do I work for a petro company, but I've served the safety board multiple times. I believe it depends on your management and how they view safety. Our management takes it very seriously, at other company's they do not. Companies don't get a violation from hard hats not being on or safety vests not zipped etc. The things you listed are not reportables. They'd also require an OSHA worker being present almost every day, watching everyone. That just doesn't happen.
yes. It isn't the employers fault at all, even though they get the safety violations while the people that actually do the job had no idea at all. Yeah, I have quit so many jobs cause I didn't want to DIE making money for some retard like Sarah Palin.
