I am only guessing, but most evidence to date seems to point toward the fact that their drilling opened up a pocket of methane gas which flooded into the mine and was not properly recognized as a problem until too late. Looks like a good reason to form another Government committee and create more regulations - and a new bureaucracy to enforce them, and new detection devices. Right?
A common problem is due to the paper feed/pickup roller being worn out or dirty it fails to grab the paper to pull it into the printer. When the paper fails to feed in, the printer assumes that there's no paper properly installed gives the paper out error. Remove the paper clean the roller with a soft cloth dampened with alcohol or warm water see if that helps. If the roller is too old or worn out, it will need to be replaced or you can try some rubber roller restorer/rejuvenator that can help old rollers.
