Puppy is STILL afraid of going outside?

I have a 15 week old cocker spaniel puppyI've had him since he was 12 weeks old - we live in a very busy street in lower manhattan and he's terrified to go outsideLately he's taking to peeing in our house whenever we grab the leash so that we won't make him go outside (we correct him when we catch him and then take him out anyway to finish).What should we do? He refuses to eat treats outside so that's no good - the street is too busy to play with a toy on (I doubt he would anyway).he is NOT getting better! I thought he would acclimate but it's not happening and he's getting to be almost 4 months old!We have tried everything - playing street noises in the house and putting treats near the speaker.putting treats in the elevator lobby on the way to going outside.literally everything! What can we do at this point?!
